Bij flegmoneuze infecties volstaan antibiotica; indien pusvorming of necrose optreedt is incisie en drainage en/of necrotomie nodig. Antibiotica-indicatie bij sepsis, verminderde weerstand (beenmergdepressie, immunosuppressiva, corticosteroïden, AIDS), verhoogd risico op metastatische infecties (b.v. patiënten met indicatie voor endocarditisprofylaxe, grote furunkels neus of bovenlip), bij onmogelijkheid tot adequate drainage, bij empyeem, peritonitis, mediastinitis, osteomyelitis en arthritis. 

Therapie:

Incisie en drainage.

R/ Antibiotica o.b.v. onderstaand schema en resistentiepatroon.

Staphylococus aureus: oraal flucloxacilline 3-4 dd 500 mg, of claritromycine 2 dd 250-500 mg, of Augmentin 3 dd 625 mg. Bij ernstige infectie: 4 dd 1 milj. E penicilline G i.v. Alternatieven bij allergie: clindamycine 4 dd 300 mg, erytromycine 3 dd 500 mg.

Streptococcen: feneticilline 4 dd 500 mg p.o. of claritromycine 2 dd 250-500 mg. Alternatieven bij allergie: erytromycine 3 dd 500 mg.

Enterococcen: amoxicilline 4 dd 1 g.

Gramnegatieve staven:: gentamycine 3 dd 80 mg, en verder op basis van resistentiepatroon.

Anaerobe bacteriën: Als Bacteroïdes fragilis niet aanwezig is penicilline G 4 dd 1 milj E i.v. of metronidazol 3 dd 500 mg. Als Bacteroïdes fragilis aanwezig is metronidazol 3 dd 500 mg of clindamycine 4 dd 600 mg. Bij Clostridia (gasgangreen) penicilline G 6 dd 1 milj E i.v. of metronidazol 3 dd 500 mg plus hyperbare zuurstof behandeling.
