Granulomateuze, vaak ulcererende ontsteking die voorkomt bij reumatoïde artritis en andere auto-immuunziekten. Synoniemen: Churg Strauss granuloma, rheumatoid neutrophilic dermatitis, superficial ulcerating rheumatoid necrobiosis (SURN), Winkelmann granuloma.

Kliniek: huidkleurig tot rood-paarse geïndureerde, pijnlijke papels, plaques of nodi, met centraal crustae en/of ulceratie. Vooral op de extremiteiten, met name strekzijde elleboog en vingers. Meestal symmetrisch. Kleine lesies kunnen conflueren tot grotere.

Histologie: ontstekingsinfiltraat met veel neutrofiele granulocyten, gebieden met necrose en collageendegradatie, omgeven door histiocyten (gepallisadeerd). 

DD: necrobiosis lipoidica, sarcoidose, erythema nodosum, reuma noduli, pyoderma gangrenosum, vasculitis, cutane m. Wegener.

 

Therapie:

R/ Lokale steroïden.

R/ Prednison systemisch 0.5-1 mg/kg

R/ Nonsteroidal anti-inflammatory drugs (NSAIDS).

R/ Dapsone.

R/ Anti-TNF-alpha (Remicade, Humira, Enbrel).
