ELASTOSIS PERFORANS SERPIGINOSA (LUTZ, MIESCHER) home ICD10: L87.2

Elastosis perforans serpiginosa (Lutz), synoniemen: elastoma intrapapillare perforans verruciformis (Miescher), keratosis follicularis serpiginosa, is een van de perforerende dermatosen die wordt veroorzaakt door een granulomateuze ontsteking en transepidermale eliminatie van bindweefsel componenten, in dit geval abnormale elastine vezels en celdebris. De laesies hebben een annulaire vorm met een grillige (serpigineuze) rand; in deze rand, opgebouwd uit papels en plaques vindt het proces van transepidermale eliminatie plaats, in het centrum, dat vaak atrofisch of verlittekend is, is dit proces al voltooid. De papels bevatten het lichaamsvreemde materiaal dat er uitgewerkt wordt; vervolgens verdwijnen ze weer. Het is een zeldzame aandoening. Het proces kan jaren duren.

Er worden 3 vormen onderscheiden:
1. Idiopathische elastosis perforans serpiginosa (geen oorzaak bekend, niet geassocieerd met syndromen).
2. Reactieve elastosis perforans serpiginosa (in het kader van syndromen waarbij bindweefsel-afwijkingen voorkomen, zoals het Marfan syndroom, Ehlers-Danlos syndroom, pseudoxanthoma elasticum, sclerodermie, osteogenesis imperfecta, Down syndroom, granuloma annulare, acrogeria, Moyamoya disease.
3. Drug-induced elastosis perforans serpiginosa (door geneesmiddelen, beschreven alleen bij D-penicillamine).

Klinisch beeld:
Huidkleurige tot rode keratotische papels en plaques, gerangschikt in een annulair, lineair, arciform, circulair of serpigineus patroon. De voorkeurslocatie is de nek, maar het kan ook op de bovenste of onderste extremiteiten, gezicht en buik voorkomen. Het veroorzaakt weinig klachten, soms wat jeuk. Het komt vooral voor bij kinderen en jongvolwassenen, in een man-vrouw verhouding van 4:1. Het kan, in zeldzame gevallen, familiair voorkomen.

Therapie:
Er zijn weinig therapeutische mogelijkheden, spontaan herstel afwachten lijkt het beste. Genoemd worden, met weinig evidence:
R/ Lokale antipruriginosa, met name menthol-producten, bij jeuk.
R/ isotretinoïne.
R/ Tazarotene 0.1% gel.
R/ calcipotriol.
R/ Intralesionale en topicale corticosteroïden.
R/ Aldara (imiquimod) crème.
mes Cryotherapie.
mes Pulsed dye laser, CO2-laser.
mes Curettage.
