NETHERTON SYNDROOM (ICHTHYOSIS LINEARIS CIRCUMFLEXA) home ICD10: Q80.8

Het Netherton syndroom (ichthyosis linearis circumflexa, Comèl-Netherton syndroom) is een zeer zeldzame, autosomaal recessieve vorm van ichthyosis die sinds de geboorte aanwezig is. Het syndroom is genoemd naar de dermatoloog Earl W. Netherton die het in 1958 beschreef. Bij de geboorte is de huid rood (erytrodermie) en droog (ichthyosis), in zeldzame gevallen is er een collodion membraan. Soms ontstaat dehydratie. Het kan spontaan in remissie gaan. Later verschijnen specifieke huidafwijkingen zoals zeer droge huid, jeuk, migrerende polycylische schilferende lesies, vervellingen aan handpalmen en voetzolen, lichenificatie in de plooien, seborrhoisch eczeem-achtige huidafwijkingen in het gelaat. Soms aminoacidurie, groei- en intelligentiestoornissen. De huidbarrière is verstoord, hierdoor ontstaan gemakkelijk infecties en allergieën. Vaak is er atopie, en het klinisch beeld kan sterk lijken op atopisch eczeem. Ook de afweer is niet goed, zowel het innate als het acquired immuunsysteem werken niet optimaal. De cellulaire immuniteit is verlaagd, de natural killer cells zijn immatuur, het IgE is vaak verhoogd. Soms gecombineerd met haarschachtafwijkingen (trichorrhexis invaginata (bamboo hairs), pili torti, trichorrhexis nodosa). De term ichthyosis linearis circumflexa beschrijft de kenmerkende huidafwijkingen die gezien kunnen worden: migratoire (wandelende, verspringende) erythemateuze plaques met een serpigineuze rand met schilfering die beschreven wordt als 'double-edged scales'. Dit onderscheidt het bijvoorbeeld van erythema annulare centrifugum, daarbij zit het loslatende schilfertje alleen aan de binnenkant van de rand (trailing scale). Ichthyosis linearis circumflexa kan in zeldzame gevallen als geïsoleerd fenomeen optreden, daarom is die term ook apart opgenomen in de diagnoselijst, maar meestal is het onderdeel van het Netherton syndroom.

Het Comèl-Netherton syndroom / ichthyosis linearis circumflexa wordt veroorzaakt door mutaties in het SPINK5 gen (OMIM 256500). Het SPINK5 gen codeert voor een serine protease inhibitor met de naam LEKTI (lympho-epithelial Kazal-type-related inhibitor). Het gevolg is dat serine proteasen in de huid niet goed geremd worden. Men neemt aan dat enzymen beinvloed worden zoals KLK5, KLK7 en KLK14 die een rol spelen in de opbouw van de epidermis en bij desquamatie.

DD: atopisch eczeem, Wiskott-Aldrich syndroom, hyper IgE syndroom, andere congenitale immuundeficiëntie syndromen.

Diagnostiek:
DNA diagnostiek, SPINK5 gen.

Therapie:
Emollientia, huid goed vet houden. Bestrijden van infecties met antibiotica. Lokale corticosteroïden hebben geen goed effect bij het syndroom van Netherton en kunnen zelfs averechts werken. Intraveneuze immunoglobulinen zijn gesuggereerd als een optie maar publicaties hierover missen nog. Ook UVB therapie en neotigason zijn beschreven in case reports. De laatste jaren verschijnen steeds meer case-reports en case-series over goede resultaten van ustekinumab (anti-IL12, anti-IL23) en anti-IL17 biologicals (secukinumab, ixekizumab, brodalumab, bimekizumab) bij het Netherton syndroom en andere vormen van congenitale ichthyosis. Bij lamellaire ichthyosis is effect beschreven van dupilumab.
