PUSTULOSIS PALMARIS ET PLANTARIS (psoriasis pustulosa palmoplantaris, ziekte van Andrews-Barber)

Als er alleen pustels bestaan, zonder verdere aanwijzingen voor een psoriasis gaat de voorkeur uit naar de term pustulosis palmoplantaris. DD: psoriasis pustulosa, (geïnfecteerd) acrovesiculeus eczeem, mycose, bacteriële (super-) infectie. Moeilijk te behandelen aandoening. Stoppen met roken helpt soms. Zie ook onder SAPHO syndroom.

R/ acidum salicylicum 5% in cremor/unguentum triamcinoloni FNA 0,1%, eventueel onder plastic occlusie.

R/ ditranol 0,1% triamcinolon 0,1% acidum salicylicum 5% in vaselinum flavum.

R/ pix lithanthracis 1-5% in zinkoxidezalf FNA.

R/ pix lithanthracis 1-5% in unguentum triamcinoloni FNA 0,1% 's nachts onder plastic occlusie.

R/ Dermovate-crème 's nachts onder plastic occlusie, 4 dagen per week of minder; cave huidatrofie aan de dorsale (!) zijde.

In zeer hardnekkige gevallen komen de volgende therapiemodaliteiten in aanmerking:

R/ Neotigason (acitretine), tetracycline of DDS (geven remming van leucotaxis), methotrexaat, PUVA, lokale (topicale) PUVA, Cyclosporine A.
