De
ziekte van Reiter (
SARA,
sexually acquired reactive arthritis,
reactieve artritis)
is een auto-immuunreactie op een infectie gekenmerkt door de volgende klassieke
trias:
1) urethritis
2) artritis
3) conjunctivitis
Bij de klassieke
morbus Reiter is de verdachte
infectie een SOA, meestal
Chlamydia trachomatis (vaak L2b serotype)
of andere verwekkers van urethritis (
Neisseria gonorrhoea,
Ureaplasma
urealyticum). En is er nog een vierde kenmerk, namelijk
mucocutane
laesies: slijmvlieslaesies (
balanitis circinata)
en/of psoriasis-achtige laesies op de voetzolen (
keratoderma blenorrhagica)
of elders. De term morbus Reiter wordt steeds minder gebruikt omdat de naamgever
Hans Reiter een fanatiek
Nazi was en kamparts in concentratiekamp Buchenwald waar hij tyfus besmettings-experimenten
uitvoerde op gevangenen. De naam is vervangen door
reactive arthritis,
en dat is een breder concept dan
SARA, want reactieve
artritis kan niet alleen door genitale infecties worden uitgelokt maar ook door
gastro-enteritis door bacteriën zoals
Shigella,
Salmonella,
Yersinia en
Campylobacter. Zeldzamere
verwekkers zijn psittacosis,
Mycoplasma pneumoniae,
Clostridium
difficile, TBC, lepra, en
Streptococcus pyogenes. De klachten
ontstaan 1-4 weken na de infectie. Circa 0.8-4% van de patiënten met urethritis
of cervicitis ontwikkelt SARA, vooral mannen (10x vaker dan vrouwen). Er is
een erfelijke predispositie:
HLA-B27 verhoogt de kans
50x, is bij 80-85% aanwezig, en een positieve uitslag is prognostisch ongunstig
t.a.v. de ernst en duur v.d. klachten. Soms is er spondylitis of iritis in de
familie. Reactieve artritis behoort tot de
seronegatieve spondylartropathieën
(reactieve artritis, ankylosing spondylitis, artritis psoriatica, IBD-geassocieerde
artropathie, juvenile-onset ankylosing spondylitis, juvenile chronic arthritis,
en undifferentiated spondyloarthritis).
Symptomen:
Genitaal:
urethritis, cervicitis, prostatitis, epididymitis, orchitis, cystitis,
vaginitis, salpingitis.
Artritis: pijn, met of zonder zwelling in 1-5 gewrichten,
asymmetrisch, vooral onderste extremiteit (knieën, enkels, voeten), soms rugwervels,
sacroiliitis.
Enthesopathie, enthesitis en fasciitis, vooral Achillespees
en fascia plantaris. Tenosynovitis en dactylitis.
Ogen: conjunctivitis, uveïtis,
iritis, keratitis, scleritis, episcleritis, cataract, glaucoom, pijn, fotofobie,
zelden visusstoornissen.
Huid: balanitis circinata penis, vulvitis circinata.
Soms afwijkingen op mucosa en tong. Psoriatiforme laesies op het lichaam, plaquevormig
of guttata psoriasis, soms pustulosis palmoplantaris. Keratoderma blenorrhagica.
Nagelafwijkingen, onychodystrofie. Erythema nodosum.
Nierafwijkingen: proteïnurie,
hematurie, pyurie, glomerulonefritis.
Hart: pericarditis, tachycardie.
Algemene malaise, koorts, vermoeidheid, afvallen bij 10% van de patiënten.
Diarree in geval van gastro-intestinale infectie als oorzaak van de reactieve
artritis.
Diversen: vasculitis, tromboflebitis parotitis, amyloidosis.
DD:Acuut reuma,
psoriasis,
psoriatiforme toxicodermie,
pustulosis palmoplantaris,
Sneddon-Wilkinson,
SAPHO
syndroom, PAPA syndroom, leukemie,
balanitis
plasmocellularis,
balanitis nno,
mucous membrane plasmocytosis,
conjunctivitis nno,
Crohn en colitis
ulcerosa,
syfilis,
Borrelia,
ziekte van Behçet,
jicht,
sarcoïdose,
SLE.
Diagnostiek:
De diagnose wordt gesteld op de combinatie van de genito-urinaire infectie,
seronegatieve spondylartropathie, extra-genitale symptomen (uveitis, mucocutane
laesies, etc.) en aanvullend labonderzoek: SOA screening inclusief HIV. IgG
en IgM tegen Chlamydia trachomatis. Reuma factor (afwezig), ANA (afwezig), HLA-B27
(60-90% pos). Grampreparaat urethra: meer dan 5 leuko’s per veld bij 1000x.
BSE, CRP, algemeen bloedbeeld, urinesediment. Lever en nierfunctie. ECG. Onderzoek
door de oogarts. X-gewrichten en wervelkolom. Consult reumatoloog. Punctie gewricht
op indicatie.
AST en anti-DNAse B
bij verdenking op streptokokken. Feceskweek (SSYC).
Therapie:
Het gaat meestal vanzelf over in enkele weken tot maanden, soms een jaar. Het
kan recidiveren. Soms ontstaat ernstige en blijvende schade door artritis. Gevonden
infecties behandelen. Gevonden soa’s behandelen volgens de richtlijnen.
Urethritis: R/ doxycycline 2 dd 100 mg gedurende
2-3 weken bij niet-specifieke urethritis, partner meebehandelen.
R/ Zithromax
(azithromycine) eenmalig 1000 mg (2 tab à 500 mg) bij aangetoonde
Chlamydia infectie, partner meebehandelen.
Balanitis circinata:R/ lokale corticosteroïden
klasse II-III.
Psoriasis en keratoderma blenorrhagica:
R/ lokale corticosteroïden klasse II-IV.
R/ Diprosalic (betamethason 0.05%,
salicylzuur 3%), tube à 50 g.
R/ calcipotriol / betamethason zalf, tube
à 60 g.
R/ Silkis (calcitriol) zalf.
R/ PUVA therapie.
R/ tetracycline
4 dd 500 mg
R/ Neotigason (acitretine) caps à 10 of 25 mg, 1 dd 25-50 mg.
R/ methotrexaat 10-22.5 mg per week.
R/ Neoral (ciclosporine) 3-5 mg/kg/dag
in 2 doses.
R/ Imuran (azathioprine) tab à 25-50-75-100 mg, 1-3 mg/kg/dag.
Conjunctivitis: Regelmatig reinigen van het
oog; in ernstige gevallen corticosteroïd-oogdruppels.
Artritis:
Ontstekingsremmers (DMARDs, NSAID’s), pijnstilling, fysiotherapie, rust bij
ernstige artritis.
R/ aspirine (acetylsalicylzuur cardio) 1 dd 80 mg.
R/ indometacine 2-3 dd 25 mg.
R/ ibuprofen 3-4 dd 400 mg.
R/ naproxen
375-750 mg per dag in 2 doses.
R/ diclofenac 2-3 dd 50 mg.
R/ fenylbutazon
caps à 100 mg, 2 dd 200 mg, onderhoudsdosering 1 dd 100 mg. Veel bijwerkingen,
wel specifiek geregistreerd voor morbus Reiter.
R/ prednison 1 dd 20-60
mg.
R/ intra-articulaire corticosteroïden.
R/ doxycyline + rifampicine;
Zithromax (azithromycine); Ciproxin (ciprofloxacine).
R/ sulfasalazine.
R/ methotrexaat.
R/ Imuran (azathioprine).
R/ Endoxan (cyclofosfamide).
R/ TNF-alfa remmers (infliximab, etanercept, adalimumab).
R/ Plaquenil (hydroxychloroquine).
