WOLVET, LANOLINE, WOLALCOHOLEN |

Wolvet (lanoline) is een complex mengsel van alcoholen en vetzuren, dat vaak toegepast wordt in zalven en cosmetica wegens de waterbindende en goed emulgerende eigenschappen. Het is een natuurproduct, bereid van schapenwol. Sebum wordt aan de wol onttrokken, gereinigd en geraffineerd tot anhydrous lanolin (wolvet). Dat bestaat uit 3 componenten: wolalcoholen, vetalcoholen en vetzuren. Wolalcoholen zitten niet alleen in zalven, crèmes en cosmetica maar ook in zepen, en vette wondgazen zoals sofratulle en fucidin intertulle.
Tegenwoordig worden er steeds meer chemisch gemodificeerde varianten en gezuiverde fracties van lanoline gebruikt, die minder allergeen zijn. Als gevolg daarvan zal het aantal patiënten met wolalcohol allergie langzaam dalen.
Op wolalcohol allergie kan worden getest door middel van plakproeven. Lanoline zit in de standaardreeks. Op de verpakking van cosmetica en zalven hoort te worden aangegeven of er wol alcoholen in zitten. Verschillende synoniemen worden gebruikt: lanoline, lanoline zuur, lanoline alcohol, lanoline was, wolvet, wolalcohol, adeps lanae, adeps lanae anhydrous, alcohol lanae, Amerchol L-101, Amerchol CAB, anhydrous lanolin, Aquaphor.
Wolalcoholen zitten niet alleen in cosmetica, maar ook in zalven, waaronder enkele FNA bereidingen zoals hydrocortisonzalf en triamcinolonzalf. Patiënten met een wolalcohol allergie kunnen ook reageren op gezuiverde en/of synthetische vetalcoholen (fatty alcohols) zoals cetylalcohol, stearylalcohol, en cetostearylalcohol (synoniemen: cetearyl alcohol or cetylstearyl alcohol). Deze vetalcoholen zijn onderdeel van natuurlijk wolvet, maar kunnen ook uit andere (petrochemische) bronnen bereid worden. Op cetylalcohol, stearylalcohol, en cetostearylalcohol kan apart worden getest, deze emulgatoren zitten in de vehiculae plakproefreeks. Cetostearylalcohol en natriumlaurylsulfaat zijn twee bestanddelen van Lanettewas SX, de basis van o.a. lanette crème, smeersel, en zalf. Cetostearylalcohol zit ook in cetomacrogol crème en zalf, patiënten die een positieve patchtest op cetostearylalcohol hebben kunnen dus ook geen cetomacrogol crème of zalf gebruiken.
