Diverse aerobe en anaerobe bacterïen

Milde infectie / oppervlakkige infectie / geen bedreigde voet
Oraal:
R/ flucloxacilline 4 dd 500 mg.
R/ clindamycine 4 dd 300 mg.
R/ amoxicilline-clavulaanzuur  3 dd 500/125 mg.
R/ cefalexine 4 dd 500 mg.
R/ ciprofloxacine 2 dd 500-750 mg.

Parenteraal:
R/ amoxicilline-clavulaanzuur (Augmentin) 3 dd 1000/200 mg.
R/ cefazoline 3 dd 1-2 g.
R/ cefradine 4 dd 500-1000 mg.
R/ clindamycine 4 dd 600 mg.

Ernstige infectie / diepe infectie / bedreigde voet
Oraal:
R/ ciprofloxacine 2 dd 750 mg + clindamycine 4 dd 300 mg.
R/ amoxicilline-clavulaanzuur 3 dd 500/125 - 1000/250 mg.

Parenteraal:
R/ cefuroxim 3 dd 1.5 g of ceftriaxon 2 d 1 g + metronidazol 4 dd 500 mg of clindamycine 4 dd 600 mg.
R/ cefazoline 3 dd 1-2 g + metronidazol 3 dd 500 mg.
R/ amoxicilline-clavulaanzuur 3-4 dd 2000/200 mg.
R/ cefoxitine 2-3 dd 1 g of cefotaxim 4 dd 1 g.
R/ piperacilline-tazobactam 3 dd 4g/500 mg.
R/ ceftazidim 3 dd 500-1000 mg.

Zeer ernstige infectie / sepsis / levensbedreigende situatie
Parenteraal:
R/ ceftazidim 3 dd 1000 mg.
R/ imipenem-cilastatine 4 dd 500 mg.*
R/ ceftazidim 3 dd 500-1000 mg + clindamycine 4 dd 600 mg.*
R/ piperacilline-tazobactam 3 dd 4g/500 mg.*
R/ vancomycine 2 dd 1 g + metronidazol + aztreonam 2-3 dd 1-2 g.
R/ flucloxacilline 6 dd 1 g + gentamicine of tobramycine 1 dd 4 mg/kg.

*tobramycine of gentamicine 1 dd 240 mg (3 mg/kg) kan worden toegevoegd de eerste 3-5 dagen.
