CALCINOSIS CUTIS NA HIELPRIK home ICD10: L94.2

Calcinosis cutis van de voetzool kan ontstaan na een hielprik (dystrophic cutaneous calcification after neonatal heel-prick / Guthrie test). De calciumdeposities ontstaan secundair aan het trauma van de hielprik (secundaire calcinosis cutis, dystrophic calcification). Een andere hypothese is dat er door de prik een kleine epitheliale inclusiecyste ontstaat, maar dan zou er histologisch een cystewand moeten worden gezien en dat is meestal niet zo.

Klinisch ziet men kleine witte of gele hard aanvoelende papeltjes met hyperkeratose op de voetzool, soms omgeven door een collerette, op een plek waar de hielprik is gezet. Als de hielprik goed is uitgevoerd is dat lateraal op de voetzool dicht bij de hiel. Solitair of multipel als er meerdere keren hielprikken zijn afgenomen. De laesies ontstaan meestal na 4-12 maanden maar kunnen zich ook pas op latere leeftijd (1-2 jaar) manifesteren. Meestal asymptomatisch, een enkele keer veroorzaken ze pijn bij lopen.

DD: palmoplantaire hyperkeratose van het punctata type, clavus, verruca vulgaris. Bij keratoderma palmoplantare punctata is er ook hyperkeratose van de voetzool, zijn er multipele laesies (veel meer) en ook op plekken buiten het hielprik gebied.

PA: dystrofische calcificaties in de dermis, de epidermis kan hyperkeratose en psoriatiforme hyperplasie vertonen.

Therapie:
Expectatief, gaat meestal vanzelf weer over in 18-30 maanden. Excisie alleen als de laesies pijnklachten veroorzaken bij het lopen. Het kan recidiveren na excisie.
